• Medical History Questionnaire (please do not mail or fax)
• Patient Check-In Form (please do not mail or fax)
• Health Insurance Card
• Diagnostic Testing Results – X-Rays, CAT Scans, MRIs, Nerve Conduction Studies
• If applicable, all Workers' Compensation information and HMO Referral Number

Preston Road Apothecary is located in the building for your convenience.

After Hours Emergencies

If you have medical questions, prescription requests, insurance inquiries or other questions that are of a non-urgent manner, you are encouraged to call the office during regular business hours. If you have an emergency after hours, you may call the office at 214.220.2468 and the answering service will direct your call to the orthopaedist on call.
